If required, we will provide an RN or LBN with the appropriate training, experience, and expertise in providing care in homes. The nurse will frequently communicate with your physician to update your care plan. Services include evaluation of patient, family members, and caregivers on disease processes; self-care techniques and prevention strategies; and coordination of patient care and services with critical health care professionals.
Through licensed therapist or therapy assistant, your therapist will provide specific information about the services and treatments you may receive. Medically necessary therapy services must be coordinated with our agency while you are receiving Medicare-covered home health services.
This will be provided by a Medical Social Worker. Services may include short-term counseling services, referral to and collaboration with community assistance and resources, with living arrangements, finances and long-term planning
We will deliver this service under the supervision of a registered nurse or licensed therapist and are provided when you need part-time assistance with personal care. These services are planned by you and your nurse or therapist and added to your plan of care. Typical responsibilities include bathing, shampooing hair, changing bed linens and assisting with other daily activities.
